Elementary students

All students entering PreK, kindergarten, Grade 1, Grade 3 and Grade 5 must have a health examination performed by a New York state licensed physician, physician assistant or nurse practitioner. The examination must be completed on the NYSED Student Health Examination Form.

A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at this time. The school will provide you with a list of dentists and registered dental hygienists who offer dental services on a free or reduced cost basis if you contact your school’s nurse.

Vision, hearing and scoliosis screenings will be completed during the school year for new students in Pre-K, Kindergarten and Grades 1, 3 and 5. All new students will receive color perception screening.

  • A copy of the health examination must be provided to the school within 30 days from when your child first starts school, and when your child starts kindergarten, Grade 1, Grade 3 and Grade 5.
  • If your child has an appointment for an exam during the school year that is after the first 30 days of school, please notify your child’s Health Office.

Middle school students

All students entering Grade 7 must have a health examination performed by a New York state licensed physician, physician assistant or nurse practitioner. The examination must be completed on the NYSED Student Health Examination Form.

Students entering Grades 7 and 8 must have the meningococcal vaccine. Without it, they can’t start school. 

Vision screenings (distance or near acuity) will be completed during the school year for new students and students in Pre-K, Kindergarten and Grades 1, 3 and 5. All new students will receive color perception screening.

Hearing screenings will be completed for all new students and students in Pre-K, Kindergarten and Grades 1, 3 and 5.

High school students

9th grade New York state requirements

All students entering Grade 9 must have a health examination performed by a New York state licensed physician, physician assistant or nurse practitioner. The examination must be completed on the NYSED Student Health Examination Form.

The school will provide you with a list of dentists and registered dental hygienists who offer dental services on a free or reduced cost basis if you contact your school’s nurse.

Students entering Grade 9 must have the meningococcal vaccine. Without it, they can’t start school.

Scoliosis screenings will be completed for all boys in Grade 9.

Scoliosis check and physical exam may be performed by private provider. If proof is not received by this office, the nurse will perform the required screening. It must be dated after the first of the school year for Grade 8 to be valid. This is required by law. NYS also requires a dental certificate.

11th grade New York state requirements

All students entering Grade 11 must have a health examination performed by a New York state licensed physician, physician assistant or nurse practitioner. The examination must be completed on the NYSED Student Health Examination Form.

A dental certificate which states your child has been seen by a dentist or dental hygienist is also asked for at this time. The school will provide you with a list of dentists and registered dental hygienists who offer dental services on a free or reduced cost basis if you contact your school’s nurse.

Health appraisal (physical) must include vision and hearing screening. If you do not want your student to be examined by the school physician, please fax over physical dated after the first day of Grade 10. Please provide all documentation to this office by 10/1. If a valid Health Appraisal is not received by this office it will be performed by the school physician. Any Health Appraisals received from your family physician must include vision and hearing screening or it will be conducted by the school nurse. A dental certificate is also required. It must be dated after the first day of school for Grade 10 to be valid. This is required by law.

12th grade New York state requirements

Students entering Grade 12 must have the meningococcal vaccine. Without it, they can’t start school.

A Menactra vaccine must be given to the student AFTER their 16th birthday. Any dose before that is invalid. Students may be excluded from school after 14 days from the first day of school if the school nurse has not been provided with evidence that the vaccine has been administered. Please fax proof of vaccine to the Health Office.
 
Students who want working papers, are ninth and eleventh graders, are new students to the district, or those who wish to play sports MUST have a physical on file less than one year old. If you’d like to use the district physician, please contact the Health Office for dates and times. It is also announced on the district calendar and district website. Please see the nurse to make an appointment.

Medicine in School

The following criteria must be met in order for students to receive prescription or non-prescription medication.

  • The school nurse MUST have on file a written order from the family physician in which he/she indicates the name of the drug, the frequency and time element for administering the medication, the dosage and possible side effects.
  • The school nurse MUST have a written request from the parent to administer the medication.
  • The medication and all refills MUST be delivered directly to the nurse by the parent, guardian or responsible adult.
  • The medication MUST be in the original container.
  • All of the above rules pertain to over-the-counter medication such as all ointments and all pain medications.

Immunization requirements are organized by grade level to help parents easily identify requirements for their child. These requirements will be updated as needed to reflect any new, mandated state immunization requirements.
